Eligibility
Unitholders aged 19-60 (age next birthday) are eligible to apply. This plan is renewable up to 70 years old (subject to yearly health declaration for renewal after age 65).

Effective Date of Coverage
The coverage will take effect upon the insurance company's approval or the bank-in date of the insurance premium.
For Personal Accident coverage, the coverage will take effect from the date of premium banked in.

Termination of Coverage
Coverage will terminate when:-
a. renewal premiums are not paid
b. a claim is made on diagnosis of a Critical Illness or upon death or total & permanent disability benefit.
c. insured attains the age of 70.
d. insured enters full-time military, naval airforce service of any country

Mutual Life Plus 2 Coverage
The Mutual Life Plus 2 is a term insurance policy which protect you against;
A. Group Term Life (GTL)
B. Group Personal Accident (GPA)
C. 36 Critical Illnesses
The benefit coverage;
- Age 19-60: Death + TPD + Critical Illness
- Age 61-65: Death + Critical Illness
- Age 66-70: Death only all causes.

A) Group Term Life (with Total & Permanent Disability) ; - GTL Coverage
a) This is a term life insurance that covers death & total permanent disablement due to natural and accidental causes 24 hours a day worldwide.
b) Total permanent disability (TPD) shall be defined as a state of incapacity of an insured member as a result of accidental bodily injury, sickness or disease such that, in the opinion of the insurance company's medical advisors, there is no reasonable possibility of the said insured member pursuing any occupation for compensation profit or gain during his remaining life-time.
The total and irrecoverable loss of the sight of both eyes, or the loss by severance of
1. both hands at or above the wrist or
2. both feet at or above the ankles or
3. one hand at or above the wrist and one foot at or above the ankle or
4. one limb at or above the wrist or ankle and loss of sight of one eye, will be considered a total permanent disability

Risk Excluded for GTL
a. Suicide within one year, while sane or insane

Risk Excluded for TPD (GTL)
The insurance shall not cover any loss or disability caused directly or indirectly, wholly or partly, by:-
a) Self-destruction or any attempt thereat while sane or insane;
b) War, declare or undeclared, civil war, revolution, any warlike operations, or state of emergency;
c) Military or naval service in time of declared or undeclared war or while under orders for warlike operations, or restoration of public order;
d) Entering, operating or servicing, ascending or descending from or with any aerial device or conveyance except as a fare paying passenger of a commercial aircraft;
e) Hepatitis B, AIDS, AIDS related complex or any other AIDS related condition.
Other benefits shall cease upon payment of the initial payment under this TPD benefit.

B) Group Personal Accident (with Total & Permanent Disability) - GPA Coverage
This is a personal accident policy that provides the insured person with a 24 hours protection against accidental injuries that shall solely and independently of any cause result in the insured's death or permanent disablement anywhere in the world.

Risk Excluded for GPA
The insurance shall not cover any loss disability caused directly or indirectly, wholly or partly, by any one of the following occurrences:
a) Self-inflicted injuries or any attempt thereat, while sane or insane, suicide;
b) Insurrection, war (declared or undeclared) or any warlike operations or military or naval service under orders for warlike operations;
c) Participation in strike, riot or civil commotion or committing an assault or felony;
d) Any form of flying except as a passenger in an aircraft licensed to carry passengers;
e) Hunting, mountaineering, racing in any form (other than on foot), winter sport and scuba-diving;
f) Any form of radioactivity or radiation;
g) Accident occurring while or because the insured is affected by alcohol or any drug unless prescribed by a licensed medical practitioner;
h) Pregnancy including childbirth, abortion, miscarriage or any complication therefrom.

The complete and irrecoverable loss of use of any member or members specified above shall be deemed to be loss of such member or members.
In the event of partial loss of any member or members specified above a proportionately lower percentage of compensation shall be payable.
In the event of Permanent Disablement by physical loss or loss of use not specified above the percentage of compensation shall be assessed in proportion to the degree of disability as compared with the cases specific without reference to the profession or occupation of the insured member.
When more than one infirmity arises from one accident the percentages are added together but cannot exceed 100% of the Principal Sum for each insured member.
In the event of a total of 100% being paid to each member all insurance hereunder shall immediately cease to be in force for each insured member. All other losses smaller than 100% having been paid shall reduce the coverage by that amount from the date of accident until the expiry of the policy of each insured member.

C) Group Critical Illnesses (CI)
Lump sum payment upon diagnosis of any one of the following critical illnesses. The payment of the Critical illness benefit is alternative and not in addition to the payment of the Death or Total and Permanent Disability Benefit.

Notes:
1. Coverage begins 60 days after the policy commencement date or reinstatement date of the policy.
2. * - Only 50% of the sum assured in force is payable. The balance of the sum assured, if in force, will be payable in the event of total and permanent disability claim or death or on the first diagnosis of another critical illness.

Risk Excluded for CI
Critical illness - no critical illness benefit is payable if
a. any covered event resulting directly or indirectly from self-inflicted injuries, while sane or insane
b. if a assured member is found to be infected at any time with Human Immunodeficiency Virus (HIV) except as provided under Benefits Provision item 1, 2 & 17 of Item C above, the assured member shall immediately cease to be eligible for cover under this rider. For the avoidance of doubt, this exclusion takes effect when the company shall at its absolute discretion shall determine that blood tests indicate either the presence of any Human Immunodeficiency Virus or antibodies to such a virus
c. With reference to Table 1 below, the company shall not be liable for any covered event benefit in List A if the assured member had been previously diagnosed either
i) as having suffered or undergone surgery for a covered event specified in List B; or
ii) as having suffered an illness specified in List C, prior to the date of this policy or this rider or effective date of rider coverage, whichever is the latest.
d. any injury or illness caused directly or indirectly, wholly or partly, by self inflicted injury while sane or insane, willful misuse of alcohol and/or the taking of drugs otherwise than under the direction of a registered medical practitioner.

Forms To Submit for Application Of New Insurance
The forms to be submitted are;
a. Proposal Form - duly completed & signed by the applicant. Please ensure that the date and agent code are written clearly on the proposal form.
b. Bank-In-Slip - bank-in-slip must be attached (must be the original carbonized copy with the computer print of the bank) with the proposal form. Please ensure that
i) the insured name, insured's IC No, agent code & the master policy no: GS8288 is written clearly on the bank-in-slip
ii) the amount is to be banked-in into Great Eastern Life Assurance (Malaysia) Bhd's PBB account: 3999205229
Kindly submit the above item (a) and (b) to Public Mutual branch for processing.

Forms To Submit For Renewal Of Insurance
The insurance premium is to be banked into Great Eastern's PBB account 3999205229. Please ensure the insured's name, insured's IC no., Certificate no. and agent code are written clearly on the bank in slip.
Kindly forward the original carbonized copy of the bank in slip to Public Mutual branch for processing.

Forms To Submit For Renewal Of Insurance (after 30 days but less than 6 months from the date of expiry)
Renewals after 30 days from the expiry date, the following must be submitted to Public Mutual Branch;
a. Bank-In-Slip - Premium must be banked-in into Great Eastern's PBB account 3999205229 and forward the bank-in-slip to Public Mutual. Insured's name, insured's IC no., Certificate no. and agent code must be stated clearly on the bank-in-slip.
b. Health Warranty Form - will have to be submitted if the expiry is more than 30 days but less than 6 months
Kindly submit the above item (a) and (b) to Public Mutual branch for processing.

Form To Submit For Renewal Of Insurance (after 6 months from the date of expiry)
a. Proposal Form - A new proposal form will have to be submitted if the expiry is more than 6 months and above. This will be considered as a new application.

Paramedic Services
In order to provide a more efficient and hassle free medical examination to Mutual Life Plus 2 (MLP2) applicants, the insurer, Great Eastern Life Assurance (Malaysia) Bhd (GE) has engaged the services of a Paramedic Service Provider, namely Asia Assistant Network (M) Sdn Bhd to service our MLP2 applicants.
The Paramedic Service Provider will make appointment with the applicant (if medical examination is required by GE) upon receipt of the authorization letter from GE. The medical requirements will either perform at the applicant's house/ office and/ or at the panel clinic or hospital.
Below is the flow chart of the whole process from arranging the medical appointment for the application to do the medical examination right up to the submission of the medical requirements to GE.
